At NDPT, we can see you with or without a physician’s referral for our services. It may be important to check with your insurance on their policy regarding physical therapy coverage.
When you come for your first visit, please arrive 15 minutes early to complete the necessary paperwork. Please remember to bring your insurance card and information with the written physician order for physical therapy treatment, if this applies. Please wear clothes to allow access to the affected area (e.g., shorts for hip, knee or ankle).
The first session will last approximately one hour, as your medical and current injury history will be taken and an initial examination will be performed. To save time on your first visit, you may download and complete the following forms and bring them with you to your initial visit:
Nebraska Doctors of Physical Therapy, P.C. is dedicated to maintaining the privacy of your information. After review of the policy, please sign the Privacy Notice Acknowledgement.
Missed Appointments
If you need to cancel your appointment we require 24 hours notice so we may offer your appointment to someone else. You may call our office at (402)502-4678. If you call after hours, please leave a message and include the date and time of the appointment you are canceling. Should you fail to notify us of cancellation 24 hours prior to your appointment a fee of $20.00 will be charged.
